Follow-up inspection required. Observed multiple food items (raw chicken, shrimp, beef) not properly date marked. Must be corrected by follow up inspection 7/12/13. Observed pooled eggs in prep line cooler, uncovered. Repeat violation. Pooled eggs must not be stored to prevent contamination. Observed accumulation debris, dust, and mold like substance on surfaces of walk in cooler and fan vents. Must be corrected by follow up visit 7/12/13. Observed cutting boards heavily scratched and soiled.

Follow-up inspection required. No proof of food safety certification at establishment. Must be present at time of follow up inspection 7/18/13. Observed bottled drink in ice machine. Corrected on site. Observed excessive amount of flies in kitchen area. Observed quat sanitizer in spray bottle over 400ppm. Corrected on site. Concentration 300ppm. No proof of employee hepatitis a records at establishment. Must show proof by follow up inspection 7/18/13. Observed accumulated debris in refrigerators/coolers. Observed soda nozzles in lobby with accumulated dirt/debris. Observed several mop heads wet and not properly hanging to dry.

Observed hand sink obstructed and not in use. Corrected on site. Items moved and soap and towels placed at handsink. Observed walls behind dish machine and above 3 comp sink with accumulated dirt. Observed dish machine with accumulated debris. 4-602.13. 3. Observed ice machine with mold like substance. Observed gasket seals in refrigerators with accumulated debris. Observed accumulated water and dirt in bar area under beer tap.
