Hepatitis A records not organized in clear concise manner. Must be corrected by next routine inspection. Observed gasket seals in refrigerators on prep line with accumulated debris, torn, and not in good condition. Observed cutting boards heavily scratched and soiled. Observed coolers/refrigerators on prep line with accumulated food/dirt debris. Observed plumbing line/drain with accumulated debris and no cover. Observed cracked light shield cover in walk in cooler.

Observed ceiling vents and tiles with accumulated dust debris. Observed fan vents in walk in cooler with accumulated debris. Observed mold-like substance accumulated near grout in dish area.

Repeat violation: Restroom doors are not self closing. Follow up required. Women's receptacle does not have a lid. Must have a lid. Floor-sticky under foot.
